New Patients

New PatientsTo ensure we provide the best possible service, new patients are asked to complete a Patient Information and Medical History Form prior to their first visit. These forms can be mailed or faxed to you, or you may download the forms here.

If you are unable to provide this information prior to your first visit, please arrive for your appointment early so that you will have time to take care of it then. New patients whose medical insurance is not verified on the first visit will be asked to pay for service in full.

Appointments

AppointmentsOur office schedules visits by appointment only, except in the case of a medical emergency. Appointments are made by calling our office at 562-946-4095 during office hours or by requesting an appointment online here.

Every effort will be made to provide the earliest possible appointment and to meet with you without delay. However, due to the unscheduled nature of emergency cases, occasionally delays may occur. We hope that you will understand that these delays are unavoidable. If for some reason you are unable to keep your appointment, please cancel as far in advance as possible. We appreciate your thoughtfulness!

Emergencies

EmergenciesCall the office phone number at 562-946-4095 any time. If the office is closed, your call will be answered by our answering service.

In the case of an emergency, Dr. Jao will be notified and will call you back as soon as possible. If this is a life threatening emergency, call 911 immediately.

Payment for Services

PaymentPayment is requested at time of service and we accept many insurance plans. If your insurance involves a deductible and/or co-payment, we will obtain it at the time of service and bill your insurance directly. However, ultimate financial responsibility rests with the patient.

If your insurance fails to pay within 30 days, you will receive a statement showing balance due which is payable within ten days of receipt. It will then be your responsibility to contact your insurance to verify their intent to pay or pay the balance yourself. Then contact our office with how you intend to handle the account. In the event that timely payment is not made, special arrangements must be made with our Credit Manager. We often accommodate unusual circumstances such as prolonged illness or unemployment. After 90 days, if no payment or arrangements have been made, necessary collection proceedings will begin.
